The CSS Recovery Coach Coordinator is a key member of the Community Based Services team, supporting the delivery of consistent, highâquality recovery coaching across our programs. This role combines direct recovery coaching with coordination and operational support, including workflow coordination, client tracking, documentation support, and onboarding of new recovery coaches. The Coordinator serves as a liaison between recovery coaches, providers, and leadership, offering mentorship and guidance (without formal supervisory responsibility). In addition, the position maintains a caseload, providing personâcentered recovery coaching, addressing barriers to care, and using lived experience to support clients in achieving their recovery goals.
Program Coordination:
- Assist in coordinating day-to-day recovery coach workflows across Clinical Stabilization Services (CSS) programs to ensure consistency and continuity of services.
- Serve as a primary point of contact for recovery coaches regarding program processes, available resources, and documentation requirements.
- Support onboarding and orientation of new recovery coaches, including workflow training and documentation expectations.
- Monitor completion and timeliness of documentation and communicate trends, gaps, or concerns to program leadership.
- Assist with data collection, tracking, and support of program reporting requirements.
- Attend program and team meetings and communicate relevant updates, changes, and expectations to recovery coaching staff.
- Support effective communication and coordination between recovery coaches and multidisciplinary care teams to promote integrated, client-centered care.
Recovery Coach:
- Provide emotional, peer-based, and social recovery support to individuals accessing Clinical Stabilization Services (CSS).
- Maintain an active caseload and deliver ongoing recovery-oriented support.
- Support individuals in identifying recovery goals and developing personalized recovery and wellness plans.
- Assess barriers to care (e.g., housing, transportation, insurance, engagement) and assist clients in navigating and overcoming those barriers.
- Facilitate linkage and coordination with healthcare providers, behavioral health services, and community-based social service resources.
- Provide overdose prevention education, including naloxone training and distribution, as appropriate.
- Participate in multidisciplinary care team meetings to support coordinated, patient-centered care.
- Conduct outreach and re-engagement activities to support individuals at risk of disengaging from services.
- Document all client encounters, outreach activities, and care coordination efforts accurately and timely in the electronic medical record.
Qualifications:
- Minimum of two (2) years of experience in recovery support services or a related field.
- Bilingual (English/Spanish) preferred.
- Must maintain strict confidentiality in accordance with HIPAA and 42 CFR Part 2 standards.
- Engage in quality improvement (QI) activities to support program performance and service excellence.
- Access to reliable transportation as travel between program sites and community locations as required to support service delivery.
Education:
- High School Diploma or equivalent required.
- Completion of a recognized Recovery Coach Academy required or ability to complete within the first year of employment.
