Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is seeking a full-time MAT Peer Adherence Worker to join our team at the Brentwood location.

Summary

The MAT Senior Peer Adherence Worker provides guidance and support to peer workers promoting consistency in collaboration of recovery support services. Has lived experience and knowledge of recovery support community resources. Provides direct recovery support services to individuals with substance use disorders including engagement, retention, and provides evidenced based interventions. Facilitates harm reduction training, and distribution of harm reduction materials.

Pay Range: $21.00 - $23.13 per hour

Essential Duties and Responsibilities

  • Provide outreach and engagement support to people who have substance use disorders.
  • Supports orientation and ongoing training for MAT peers.
  • Submits requested reports completed and on time.
  • Maintains professional competence through participation in staff development and educational programming.
  • Conducts chart review to verify accuracy of reporting.
  • Completes recovery support plans with patients.
  • Facilitates Harm Reduction Trainings including but not limited to Opioid Overdose Prevention, Fentanyl Test Strips, Xylazine Test strips and other harm reduction aspects to care.
  • Completes care plans with patients.
  • Knowledge of and linkage to recovery support services
  • Participation in quality improvement projects to enhance peer and program services.
  • Provides guidance and support to peer workers ensuring consistency in recovery support tailored to the unique needs of each individual patient.
  • Maintains visibility and positive relationship with site staff promoting increased interdepartmental collaboration of care for patients who have substance use disorder.
  • Provides direct recovery support utilizing the eight dimensions of wellness and evidenced based models of care.

Requirements

  • Minimum Education: High School Diploma/GED
  • Preferred Education: Associate’s degree, Certified Recovery Peer Advocate (CRPA)
  • Minimum Work-Related Experience:1 year
