General Description of Position:  The primary responsibilities of the Extender are to support the Care Managers in delivering Tailored Care Management to the members. Extenders support members by providing information about and assistance with accessing services and community resources through life experiences to improve the individual's health outcomes. The Extender supports care management efforts through a combination of activities with various public and private agencies, development of processes and agreements to address critical community needs of individuals enrolled with the Tailored plan, foster relationships and promotes whole person, integrated care by planning, coordinating, tracking, and monitoring care to individuals to achieve the key goals of the behavioral health tailored plans.

 

Duties and Responsibilities:

  • Member Engagement: Reach out to members, establish rapport, and assist in gathering necessary information for care planning.
  • Care Coordination: Assist in coordinating appointments, services and transportation for members. Ensure members receive necessary reminders and follow-ups.
  • Data Management: Maintain accurate and up-to-date records of member interactions, appointments and services provided.
  • Team Collaboration: Work closely with Care Managers and other team members to ensure seamless communication and efficient care delivery.
  • Resources Assistance: Provide information and assistance to members in accessing community resources and social support services.
  • Supportive Role: Share personal experiences sensitively when appropriate, providing a relatable perspective for members.
  • Documentation: Complete all required documentation accurately and in a timely manner.
  • Training and Development: Participate in training programs to enhance skills and stay updated on best practices in care management.
  • Adherence to Policies: Follow all organizational policies and procedures to ensure compliance with regulator standards.
  • Other Duties: Undertake additional tasks as directed by the Care Manager or supervisor to support the overall objectives of Tailer Care Management.

Qualifications

Qualification/Education/Experience:

  • A minimum of a high school diploma or equivalent
  • Individuals must be at least 18 years old.
  • Preferred: NC Certified Peer Support Specialist
  • Previous experience in a healthcare or community support role preferred.
  • Excellent interpersonal and communication skills
  • Ability to work collaboratively within a team and independently when necessary.
  • Empathy and understanding of individuals with behavioral health conditions.
  • This role will require supervision from a Care Manager.
  • Must meet one of the below requirements:
    • Be a person with lived experience with a behavioral health condition who is a NC Certified Peer Support Specialist
    • A parent or guardian of an individual with a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (parent/guardian cannot serve as an extender for their family member).
    • Has two years of paid experience performing the types of functions described in the Responsibilities section of this job description, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
  • Individuals who will be able to meet these requirements are, including, but not limited to the following:

      • NC Certified Peer Support Specialists.
      • Community Health Workers, defined as individuals who have completed the NC Community Health Worker Standardized Core Competency Training (NC CHW SCCT)
      • Individuals who served as Community Navigators prior to the implementation of Tailored Plans
      • Family Navigators, as defined by Trillium health Resources’ approved LME/MCO in lieu of service description.
      • Parents or guardians of an individual with a behavioral health condition (parent/guardian cannot serve as an extender for their own family member); and
      • A person with lived experience with a behavioral health condition
