TASKS AND RESPONSIBILITIES

  1. Treat all individuals with dignity and respect while maintaining agency standards of confidentiality, professionalism and objectivity.
  2. Adhere to all policies and procedures set forth by Prairie Ridge. Complete all required documentation in accordance with agency, funding, and billing requirements.
  3. Maintain professional boundaries with clients, staff and other outside agencies.
  4. Regularly utilize skills such as problem solving, quality management, sound and ethical judgement, planning, organizing and interpersonal skills.
  5. Provide oversight of the day to day activities of the Drop-In Center
  6. Assist in evaluating and selecting programs that produce results
  7. Develop and maintain relationships with community agencies, organizations, and service providers to support referral and resource opportunities for individuals served.
  8. Participate in various workgroup and committee meetings to support departmental and organizational efforts to improve the quality of services throughout our systems of care.
  9. Develop and maintain a working knowledge of all documentation, reports, processes and state requirements.
  10. Build and maintain collaborative relationships with community partners to promote and enhance Drop-In Center services and community engagement.
  11. Coordinate care and communicate effectively with Prairie Ridge programs and departments to support continuity of services.
  12. Help patients with paperwork and entitlement applications
  13. Provide education and training to individuals, groups, and agencies.
  14. Provide monthly supervision for all staff directly supervised;
    1. including approving timesheets and time off requests.
    2. Monitoring staff performance and addressing concerns in a timely manner; Creating performance improvement plans for staff when necessary
    3. Provide regular documentation for supervision
    4. Provide supervision, mentorship, coaching, and role modeling to peer support staff.
  15. Provide direct and indirect support and education to Drop-In Center patients and their families. This requires the ability to:
    1. Maintain a regular schedule and hours at the Drop-in Center that are convenient and appropriate to client/family needs
    2. Effectively assess the needs of client/family
    3. Maintain knowledge of community resources, eligibility requirements, and points of access to effectively guide individuals and families.
    4. Develop and maintain healthy, supportive relationships with clients/family.
    5. Assist individuals with service navigation, referrals, and linkage to behavioral health, physical health, housing, employment, and other community supports.
    6. Provide exceptional customer service while fostering positive working relationships with patients, families, providers, staff, and community stakeholders.
    7. Record and maintain documentation in Electronic Health Records, maintain patient confidence and protect operations by keeping information confidential.
  16. Other duties as assigned.



QUALIFICATIONS

  1. Bachelor’s Degree in a human services field (preferred); 5 years of related experience/training; or an equivalent combination of education and experience. 
  2. Lived experience accessing mental health services for self
  3. Minimum 2 years of experience in a leadership or supervisory role (preferred)
  4. Understanding of the mission of Prairie Ridge and patients we work with.
  5. Must possess a valid driver’s license, liability insurance policy, and a reliable means of transportation.
  6. Peer Support Specialist certification or the ability and willingness to obtain certification during the first six (6) months of employment.
  7. WRAP trained or the ability and willingness to obtain WRAP training during the first available training post hire.
  8. Proficiency in oral and written communication skills, as well as recordkeeping.
  9. Technology skills allowing for proficiency in use of electronic health record.
  10. While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel objects, tools or controls; and talk or hear. The employee may be required to walk and stoop, kneel, crouch, or crawl. The employee is often required to stand, reach with hands and arms, and climb or balance.
  11. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or more up to 35 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, and the ability to adjust focus.
