JOB SUMMARY
Integrated Peer Models for Health Program (IPMH) is an established program of the Health Federation of Philadelphia that’s funded by the Pennsylvania Department of Human Services to utilize and support Community Health Workers (CHWs) in serving populations facing multiple barriers to health and wellbeing, including behavioral health challenges, social drivers of health, and chronic disease, within the North Philadelphia Health Enterprise Zone (HEZ).
The Program Coordinator will work with the IPMH Program Director and partners on the next iteration of this grant and will play an important role in supporting all aspects of the program’s administration and implementation. Program activities will include leading work with partner organizations on the utilization of Connect4health, a resource and referral platform, coordinating professional development activities for our CHW and Supervisor learning collaboratives, and managing the roll out of a new CHW direct-service initiative, hiring CHWs to support behavioral health needs within partner organizations under the direction of the Program Director.
To be successful, this individual must be: a reliable, self-starter with strong project coordination, organization, administration, and communication skills, who is collaborative, and adept at learning new processes and technologies.
This position is full-time and supported by grant funding for the next 2-years.

Responsibilities/Duties
Education: Master’s degree in Social Work, Public Health or other relevant study areas preferred, or Undergraduate degree with required skills and experience.
Skills/Experience
Work Environment: The Program Coordinator will work in a 60/40 hybrid format (60% of the time in-office and 40% of the time virtual) and will report to the Health Federation of Philadelphia, 123 S. Broad Street, Suite 650, Philadelphia, PA 19109. Various meetings and training will be scheduled off-site throughout the city of Philadelphia.
Position Type and Work Schedule: Full Time, 37.5 hours per week. Typical hours are Monday through Friday 8:30 am to 4:30 pm, but candidate must be willing to work a flexible schedule, including periodic evenings and weekends, as needed, with notice. Flex office schedule options available with supervisor approval.
Continued funding for this project is not guaranteed.
Travel: Some local travel to meetings and events at various sites may be required each month. Candidate must be able to effectively travel within Philadelphia and surrounding counties, either via public transportation and/or in own vehicle.
Physical Demands: This position requires someone who can transport materials and move equipment or supplies weighing up to 30 lbs.
